<p>Dr. Chilkov:    As we age, our metabolism slows down, and there are many changes in our hormonal balance. We have to look at changes in estrogen, insulin and thyroid function as well as changes in lifestyle. Women can get more sedentary as they age because of the demands of life.</p>
<p>All of these factors will cause an increase in body fat. It&#8217;s lovely to be thin for cosmetic reasons and vanity. But really, we want to have low body fat and more muscle mass. High body fat increases our risk of cancer, cardiovascular disease, arthritis and depression. So many health issues are made worse by having a higher percentage of body fat. That should be our motivation for being leaner and thinner.</p>
</p>
<p>Dr. Chilkov: The bottom line is having a lifelong normal weight. Maintaining a normal fat-to-lean-body-mass ratio is a lifestyle. Then we deal specifically with metabolic issues with herbs and nutrients.</p>
<p>You have to include the lifestyle factor of regular exercise. If you want to maintain your weight, you want to exercise an hour three times a week. If you want to lose weight, you have to do that every day. You want to have a healthy diet and proper stress management &#8212; stress hormones cause your body to store fat. And you want to have adequate rest, so you aren&#8217;t swimming upstream against your own physiological factors.</p>
<p>There are a number of herbs and nutrients that we can use to help our body burn more fat, even while we are sitting. Let me introduce some of those.</p>
<p>The first one that I want to discuss is CLA; that is an abbreviation for conjugated linoleic acid. This is a type of fatty acid that promotes fat loss and muscle growth. It occurs in some foods &#8212; for example, it is very high in cottage cheese. I prescribe 1,000 milligrams, twice a day, in a capsule as part of a weight-loss program. It increases the burning of fat. That is how it helps us lose weight more efficiently. We are going to promote fat loss, but we also are going to promote muscle growth. Why is that important? Muscle has a higher metabolic rate, so it keeps us lean. If you have more muscle mass, you can eat more calories and still stay thin. You want to be using things that not only burn fat but also build muscle. That is a real key to staying lean.</p>
<p>CLA is used in treating other health concerns. It&#8217;s used in cancer care, in treating cardiovascular disease and diabetes. It&#8217;s not just used as a weight-loss supplement.</p>
<p>A lot of people have heard a lot about Green Tea and weight loss. It is a useful herb for weight loss because it, too, will improve fat metabolism. Green Tea also reduces appetite. It has a little bit of caffeine and the main medicinal component, ECGC. The decaffeinated form is not as effective for weight loss as the natural caffeinated form. It can be taken in capsules, or it can be taken as a beverage tea. When you take Green Tea in capsules, you want a fat-soluble capsule; it is much more effective. Green Tea really, really increases thermogenics, or energy expenditure (calorie burn), that can boost your metabolism by 4 percent a day.</p>
<p>Green Tea also helps keep blood sugar stable. That is why it reduces appetite. Keeping blood sugar stable is a big component of weight loss. If your blood sugar drops, your body will produce the stress hormone cortisol. That is a fat-storage hormone. Keeping your blood sugar stable during the day by eating small, frequent meals and not waiting too long to eat really makes weight loss much more efficient.</p>
<p>Green Tea is a fabulous antioxidant. It protects your cells from damage. Green Tea is also wonderful in cancer treatment and cancer prevention. There are many benefits to using Green Tea besides weight loss.</p>
</p>
<p>Dr. Chilkov:    When you buy teas, herbal medicines or nutritional supplements, you want to buy the absolute highest quality. Buying a cheap brand is not the best way to get a real therapeutic outcome.</p>
</p>
<p>Dr. Chilkov: There are two more that I wanted to discuss. One is carnitine. Carnitine is the main amino acid in our muscle. It is also very important for our brain. But carnitine is required for fat metabolism. Sometimes vegetarians have trouble with weight loss because they are carnitine-deficient. They have very little carnitine in their diet because carnitine primarily comes from flesh foods.</p>
<p>Carnitine is another supplement that increases fat-burning and builds muscle. There are certain groups of people who really want to be careful about losing too much muscle &#8212; for example, the elderly, people who have chronic illnesses, and people who have cancers. When we want to build muscle, we put carnitine in the body. We can also use it when we want to lose fat. Carnitine enters a pathway in the cell that is part of the energy factory inside the cell. If you push that forward, you are going to burn more fat. Carnitine is required for that pathway in our cells. Without carnitine, we cannot burn fat for energy. Therefore, carnitine will benefit people who are suffering from fatigue as well as obesity. Carnitine is also helpful in treating some forms of depression.</p>
<p>About 800 to 1,000 milligrams three times a day is the dose I recommend. Carnitine can be taken in a powder form or a capsule form. Some supplement companies even make it in a liquid form, which I have found is useful in children.</p>
</p>
<p>Dr. Chilkov:    There&#8217;s really not a lot of benefit from taking double or triple the doses. You want to stay in a therapeutic dose range, where you are going to alter your metabolism in a healthy way. I want to emphasize that what we want is healthy weight loss. And at the end of the weight loss, we want a healthy woman. If you lose weight in a healthy fashion, you will also keep it off. That will relieve you of yo-yo dieting.</p>
</p>
<p>Dr. Chilkov: I have one more that is not well known. It&#8217;s Coleus forskohlii, which is a plant. The main active constituent in that is called forskolin. Again, it increases your metabolism and thermogenesis &#8212; fat-burning, calorie burning and energy expenditure. The other thing about Coleus is that it will stimulate your thyroid. Of course, that will increase metabolism and weight loss as well.</p>
<p>Coleus also has another benefit in that it regulates insulin and blood sugar. I want to say a little bit about that because regulating insulin and blood sugar is another key to weight loss. CLA, which I mentioned at the beginning of this talk, also does that. You asked me why we gain weight around the middle as we age. The reason that happens is because a syndrome called insulin resistance develops. Insulin, the hormone that unlocks the door for sugar to go inside the cells, starts to become less efficient. We start to store our calories as fat rather than burn them as energy. When you use something like CLA or Coleus, you&#8217;re counteracting that pattern. That pattern develops more in people who abuse sugar and eat too many sweets. By limiting carbohydrates and sweets, you stop yourself from developing this fat storage around your abdomen due to insulin resistance. If you have a history of diabetes in your family, you are going to be much more prone to something like this.</p>
<p>Coleus is also a slight mood elevator. That, of course, will affect food cravings, our motivation to exercise and eat right. By dealing with things that affect our mood, levels of stress, anxiety and depression, we will be able to engage in healthy choices that cause us to be lean and to lose weight. I think it&#8217;s important to include things in your plan to address that aspect.</p>
</p>
<p>Dr. Chilkov: Typically, you will see it in capsule form. You will see weight-loss products that contain all of these ingredients. There are a number of good products out on the market, or variations of these. These four things that I have talked about can be used individually or together.</p>
<p>To summarize, what you want to do in a weight-loss program is make sure you are eating a diet that is appropriate for you. You do not want to be on an extreme low-calorie diet. You want a good balance between your proteins, carbohydrates and healthy fat. (Older women do better by limiting their carbohydrates. They will lose more weight by doing this because of the insulin relationship.) You want to have regular exercise. You want to make sure that you get adequate rest, keep your blood sugar stable throughout the day, and manage your stress well. If you do all of those things, you will become lean.</p>
</p>
<p>Dr. Chilkov:    If you set up some structure for yourself and stick with it, this is not extreme. This is very doable.</p>

<p>I always like to start out by stating I am not a doctor, I am not offering cures or medical advise. I am a concerned citizen who is interested in researching alternative options for health and wellness.</p>
<p>Since the 1930&#8242;s, American women have been trained to think that a &#8220;natural&#8221; part of growing older is Menopause, this &#8220;disease condition&#8221; requiring treatment. If Menopause is a disease, how did millions of women go through history without Hormone Replacement Therapy (HRT) drugs to deal with it? How do women in third world countries handle going through it without HRT&#8217;s?</p>
<p>Diet plays an enormous part in how we &#8220;go through&#8221; Menopause. For instance, Shangri-La place in the Himalayas called Hunza Land, famous for longevity to 120 years old, symptoms of Menopause were unheard of.</p>
<p>Natural Phytoestrogens (plant-estrogens) are found in plants like, licorice, soybeans, alfalfa, and many others. Phytoestrogens are weak estrogens and have the ability to block the stronger forms.</p>
<p>In 1966 a best seller, Feminine Forever, by Robert Wilson, a New York Gynecologist, explained that Menopause is an Estrogen- dificiency disease. Insufficient Estrogen supposedly caused women to lose her youth, beauty, cheerful attitude, vaginal dryness, hot flashes, bone density, all at once, with the onset of Menopause. The drug industry in a heart beat donated .3 million to set up the Wilson Foundation for the sole purpose of developing and promoting Estrogen drugs. The FDA&#8217;s approval for sythetic Estrogen was given based on one study. Throughout 1964 and 1965, fueled by advertising powers, Estrogen Replacement Therapy (ERT), a synthetic Estrogen was being popularly prescribed.</p>
<p>In 1975 a little snag came up, findings of increased Endometrial Cancer cases had gone up by 5 times. Unless they had been using the drugs for longer than seven years, then it was 14 times the normal incidence.</p>
<p>WHAT IS ESTROGEN?</p>
<p>Estrogen is really a general term for three seperate Hormones, Estriol, Estradiol and Estrone.</p>
<p>Estrogen is produced in three main places in a woman&#8217;s body, in the Ovaries, Adrenal Glands and the Fat Cells. </p>
<p>What is Estrogen&#8217;s Main Purpose? The main purpose of Estrogen is the make the uterine lining, the endometrium, ready to implant a fertilized egg in the event fertilization occures. To aid in this function, Estrogen will promote&#8230;water retention, fat storage and maturation of the female adolescent. This is all fine, if pregnancy is likely, but EXCESS Estrogen throws off the timing. Excess Estrogen causes the body to prepare for embryo implantation all the time. This state of over-preparation is the cause of , sluggish blood circulation, migraines, increased clotting, high stroke risk, fibroids, endometriosis, weight gain and a host of other conditions as well as disrupted copper/zinc ratios in brain cells attributing to mood swings.</p>
<p>Estrogen has a sister hormone called Progesterone, whose functions are equally important.</p>
<p>What is Progesterone? Progesterone is the other primary female hormone. It is produced in the ovaries. It is the precursor for both Estrogen and Testosterone. Progesterones functions include, maintaining the endometrium in pregnancy, new bone formation, regulation of blood pressure, fat conversion, sugar metabolism, maintaining myelin sheath (nerve insulation) and regulates the Estrogen Production.</p>
<p>When an egg is presented once a month from the ovaries, wrapped in an envelope called a follicle. After the follicle lets go of the egg, the egg journeys down the Fallopian tubes on its way to the uterus, where it awaits possible fertilization. The burst follicle still has an important job to do: it begins to produce Progesterone, for the next two weeks. Progesterone&#8217;s job is to maintain the uterine lining until one of two things happen. Pregnancy or no Pregnancy, if Pregnancy occurs Progesterone production is taken over by the developing lining itself &#8211; the Placenta. If no Pregnancy occurs, the follicle stops producing Progesterone, which triggers the collapse of the blood-rich lining, which is then expelled as the woman&#8217;s monthly cycle.</p>
<p>So, these two hormones are responsible for entire infastructure of reproduction. So what then is the problem?</p>
<p>Back to Estrogen Dominance. If Estrogen is too high, Progesterone cannot keep the dynamic of balance. How does Estrogen get too high? Overrefined diets, external toxins from the environment, (xenoestrogens), and lack of excersise. These can raise abnormal levels of Estrogen. Lets focus on Xenoestrogens, or &#8220;foreign&#8221; Estrogens. Many external Toxins have been found to have Estrogen-like effects in the body, most are from Pesticides and Fertilizers, Petroleum Derivatives, Plastics, Soaps, (foaming agents, hormone laced meat and milk products, etc. This affects everyone on a daily basis, men, women and children. The research has shown that high levels of Estrogen in Men show Gonad shrinkage as well as up to a 50% drop in sperm count, in Women, overdeveloped Ovaries as well as the host of problems listed earlier and in children early onset of Puberty. Progesterone is simply overwhelmed by the dominance of Estrogens.</p>
<p>As Estrogen levels build up to twice the normal level, many systems of the body are adversely affected. Our Body Fat stores Estrogens. Estrogen Dominance can cause Sugar Metabolism Defects leading to Diabetes, Endometrial Cancer is increased by 5-14 times, Osteoporosis, Toxemia ( blood poisoning), leading to autoimmune disorders such as, Lupus, Chronic Fatigue, and Arthritis. The increase risk of Stroke is 50% with extrogen, according to an extensive project, known as the Boston Nurses Questionnaire Study, of 121,000 nurses. Estrogen Dominance can lead to Fibroids, Breast Cancer and it has a detrimental affect on B Vitamin depletion, leading to fatigue, sluggish memory and hair loss. &#8220;The only known cause of Endometrial Cancer is unapposed Estrogen,&#8221; Dr. Lee, author &#8220;What Your Doctor May Not Tell You About Menopause.&#8221;</p>
<p>A growing number of medical researches who do not represent interests of the drug cartels are stepping forward to show that the symptoms of menopause are not caused by too little Estrogen, but by too much. American women arguably are among the most stressed people in history, emotionally and nutritionally, as well as environmentally. It is well known that stress depletes the Adrenal Glands. When this happens, Progesterone is converted to Adrenal Hormones, like Cortisol, to take up the slack and try to keep up with Adrenal demands. Remember Progesterone is their precursor. The result is further depletion of Progesterone, again promoting Estrogen Dominance.</p>
<p>Hormone Replacement Therapy, is this the right option?</p>

Synthetic Drugs, even ones that include &#8220;Synthetic Progesterone&#8221; is not handled by your body like a &#8220;Natural Progesterone,&#8221; one that is bio-identical to your bodies own Progesterone Hormone.</p>
<p>What Solutions are available to combat Estrogen Dominance?</p>

<p>Many believe that the transition into menopause is a turbulent rollercoaster; however, this doesn&#8217;t always have to be the case. It&#8217;s true! I&#8217;ve had women in my practice enter menopause with minimal to no &#8220;typical menopausal symptoms.&#8221;</p>
<p>The fact is that symptoms are just your body&#8217;s way of communicating that something isn&#8217;t right. If the body had what it needed, it wouldn&#8217;t be making us so uncomfortable in order to get our attention! Therefore, symptoms of menopause can actually serve as a guidepost to what is going wrong. As a Naturopathic and Functional Medicine Practitioner, I want to know why the body is signaling this imbalance, and then once this cause is addressed, the symptoms subside.</p>
<p>For example, hot flashes are related to fluctuations in hormones. This is apparent at menopause because the adrenal glands are taking up the slack of the decreased production of estrogen and progesterone from the ovaries. Therefore, if a woman is already stressed out in everyday life, menopause will present with noticeable hot flashes. However, hormonal surges will be minimal if her body is already strong and healthy.</p>
<p>If a woman is experiencing hormonal symptoms, support can be given to help her regain balance through lifestyle, nutritional support, and adrenal and hormonal balancing herbals. If estrogen is low, herbals such as red clover, black cohosh, or other phytoestrogens can be used. Progesterone support can be found in other herbals such as wild yam.</p>
<p>Still, it&#8217;s important to assess what the cause of the hormonal balance is. We already discussed adrenal stress, but in my clinic, I not only look at hormonal balance but also blood sugar fluctuations, gut disturbances, liver support, and inflammation.</p>

<p>&#13;When dealing with estrogen dominance, it is not a good idea to take more progesterone to even out hormonal balance. Taking more progesterone won&#8217;t necessarily restore hormonal balance. Rather, it might be metabolized into more estrogen, resulting in an even greater hormonal imbalance and greater estrogen dominance. Instead, one should take the DIM supplement, which improves hormonal balance and eliminated estrogen dominance by shifting the way that estrogen is metabolized in our body. This creates more &#8220;good&#8221; estrogen metabolites and less &#8220;bad&#8221; estrogen metabolites. Although it does not increase or decrease estrogen levels, it directs how the estrogen is metabolized.</p>
<p>&#13;Research has suggested that DIM may also help to prevent breast cancer. After studying a group of women for periods up to nineteen years, the women who developed breast cancer were noted to have a significantly lower ratio of &#8220;good&#8221; estrogen metabolites as compared to &#8220;bad&#8221; estrogen metabolites. In comparison, those who did not develop breast cancer had a high ratio of &#8220;good&#8221; estrogen metabolites to &#8220;bad&#8221; estrogen metabolites. Scientists have determined that DIM actually inhibits the growth of cancer cells. It enhances the activity of enzymes found in certain pathways of the liver, which are important for the healthy estrogen metabolism that reduces the risk of cancer development.</p>
<p>&#13;When buying a DIM supplement, it is suggested that you read the label of the DIM product you&#8217;re going to buy. Be certain that you are getting 120 mg of DIM complex, which is standardized to 30 mg DIM per dose. Because DIM is very hard to absorb, the products needs to be in a specialized complex that improves bioavailability.  Although most women only need one dose of DIM per day, women suffering with a large amount of estrogen dominance symptoms may find greater relief by doubling the dose and taking DIM twice a day. Because some women may have a minor stomach ache after taking DIM, taking the supplement with food will eliminate the problem. <p>Much too often I get asked about the statement &#8220;she doesn&#8217;t need progesterone if she doesn&#8217;t have a uterus&#8221;. It is hard to conceive that medical schools are still teaching the idea that a woman without a uterus does not need progesterone. It&#8217;s also hard to believe that large number of practitioners consider this statement true without questioning it. It is not supported by physiology, biochemistry or science.</p>
<p>Synthetic progestins do not imitate well the actions of progesterone outside of the uterus. In fact, medroxyprogesterone acetate (MPA), the most frequently prescribed synthetic progestin in the U.S., produces the exact opposite effects of endogenous progesterone in almost all, if not all, organs or systems outside of the uterus. The drug first given to us to mimic the effects of natural progesterone works only like progesterone in one organ and everywhere else produces the exact opposite effect of progesterone! </p>
<p>This misguided philosophy results in practitioners not giving progesterone or progestins to patients who have had a hysterectomy, and therefore practitioners never see a direct comparison of the effects of the two different agents on the rest of the body. The above statement helps to keep practitioners from comparing the known proven effects of progesterone on many body systems to the results caused by administration of the &#8220;progesterone-mimicking&#8221; synthetic progestins. If practitioners would look at the lack of science behind the statement, and the body of evidence supporting overall health benefits of progesterone itself, synthetic progestin use would most likely plummet.</p>
<p>Limiting the use of progesterone to women who have a uterus ignores the science that progesterone has beneficial effects outside of the uterus. Progesterone receptors have been identified in almost every cell in the body; therefore, progesterone has functions throughout the entire body. A quick look at the evidence shows many favorable effects of progesterone, whereas MPA produces unwanted side effects or risks which are the exact opposite of the actions of progesterone.</p>
<p>Breast cancer risk is a chief concern for the majority of women considering <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link/3657162']);" href="http://www.zrtlab.com/download-document/157-lifeafterhrtpatienthandout.html">hormone replacement therapy</a>. Evidence overwhelmingly shows that any synthetic progestin use, even for a limited time, increases the risk of breast cancer. The longer the use of progestin, or the more testosterone-like the synthetic progestins, the greater the risk becomes. The studies may disagree on how high the risk increase is, but they all agree that synthetic progestins significantly increase the risk of breast cancer. The mechanisms have been identified for the most part, and they are opposite to the mechanisms of endogenous progesterone in breast tissue.      </p>
<p>In opposition to the increased risk of synthetic progestins, <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link/3657162']);" href="http://www.zrtlab.com/download-document/153-bioidenticalhormonespatienthandout.html">bioidentical progesterone</a> has been shown to possess anti-proliferative and anti-cancer properties. A low level of endogenous progesterone is associated with an increased risk of breast cancer. Clinical studies suggest progesterone therapy protects against breast cancer. Some studies have even shown a decreased risk of breast cancer when natural progestin is added to estrogen therapy.</p>
<p>Speroff&#8217;s textbook, Clinical Gynecologic Endocrinology and Infertility, states that the proliferative action of estrogens and the progesterone actions of differentiation and apoptosis are the same in the breast tissue as in the uterus. Therefore, progesterone, and only progesterone, can protect the breast tissue from cancer by the same mechanism that any progestin protects the uterus. Looking at the science, the above statement needs to be modified to &#8220;she doesn&#8217;t need progesterone if she doesn&#8217;t have a uterus or breasts&#8221;.</p>
<p>The number one killer of women over the age of 50 is cardiovascular related events. MPA has been shown to completely negate the favorable effects of estrogen on the maintenance of a healthy lipid profile and the cardiovascular benefits of estrogen, and significantly increase cardiovascular risks. In contrast, progesterone works synergistically with estrogen to increase lipid maintenance and cardiovascular health. Based on the difference between MPA and progesterone, the writers of the largest hormone study at its time, the 1995 PEPI study, concluded that &#8220;we should be giving woman natural progesterone&#8221; (this conclusion has still to be incorporated into mainstream medicine). Based on the evidence, the statement needs to be modified to &#8220;she doesn&#8217;t need progesterone if she doesn&#8217;t have a uterus, breasts or a cardiovascular system&#8221;.</p>
<p>Other major health concerns for the aging female patient are the development of osteoporosis and dementia. Progesterone works synergistically with estrogen to provide better bone remodeling through stimulation of bone growth, while progestins do not. While progesterone provides neuroprotective benefits for the nervous system, side effects of synthetic progestins are the opposite. Now we need to correctly state, &#8220;she doesn&#8217;t need progesterone if she doesn&#8217;t have a uterus, breasts, a cardiovascular system, bones, or a brain&#8221;!</p>
<p>Additional evidence continues to demonstrate the opposite effects of progesterone vs. the synthetic substitutes. MPA is proliferative and inflammatory; progesterone has been demonstrated to be neutral or slightly anti-inflammatory and anti-proliferative in its actions. Progesterone is the drug of choice for protection from further damage in traumatic brain injuries, and in at least two small studies progesterone has been shown to help reverse the debilitating effects of brain injury. MPA causes degenerative changes in the liver, while progesterone does not. Physiologic levels of progesterone augment the release of insulin from the pancreas, while synthetics increase insulin resistance.</p>
<p>The evidence clearly indicates that bioidentical progesterone provides a number of protective benefits not offered by synthetic progestins. Synthetic progestins instead increase risks associated with loss of these protective benefits. Therefore, I would recommend that our health professional educators examine the evidence and teach what the scientific evidence demonstrates: a woman does not need a progestin if she does not have a uterus, but she requires progesterone when it becomes deficient in perimenopause to continue to provide protection of her breasts, heart, blood vessels, bones, nervous system muscles, liver, skin, and perhaps much more yet to be discovered.                </p>

<p>The term post-menopausal sometimes needs a little clarification.  It is usually applied to women who have completed the menopause, meaning they have not had a period for over twelve months. This can be due to a natural menopause, or a surgery induced menopause. Women who are taking hormone replacement therapy are not considered to be post menopausal as they may experience symptoms of the menopause when they stop this treatment.   </p>
<p>&#13;<br />
Common post menopausal problems can be experienced any time from the onset of the menopause onwards.  After the menopause, women never really stop being post menopausal. Most of the physical and emotional problems that women encounter after the menopause are caused by decreasing hormone levels, particularly estrogen. </p>
<p>&#13;<br />
Conditions relating to the vagina and bladder are often experienced post menopause because these parts of the body require estrogen to remain healthy.  Thinning of the tissue in these areas can cause incontinence, bladder infections, growths or polyps in the uterus, and vaginal dryness, as well as an increased risk of uterine cancer.      </p>

Other serious physical conditions linked to post menopausal women are osteoporosis and heart disease.  Osteoporosis is common as women begin to lose bone density more rapidly due to reduction in estrogen.  Risk of heart disease is increased due to atherosclerosis, or the hardening of their arteries. </p>
<p>&#13;<br />
Emotionally, post menopausal women may experience depression, fatigue due to insomnia, hot flashes and night sweats, and a reduction in self confidence and libido.  Although these emotional responses can be triggered by hormonal changes, they may also be related to other factors.  Reaching the menopause, and the changes happening in her body, may make a woman feel old, unattractive and worthless and these feelings can trigger depression and lack of sex drive. </p>
<p>&#13;<br />
So what can post menopausal women do to limit these common problems?  Exercise is one crucial way to control these conditions.  While we are not talking about running marathons, some exercise such as walking, gardening and light aerobics can have a wide range of benefits.  </p>
<p>&#13;<br />
Exercise keeps bones, joints and cartilage healthy, guarding against osteoporosis as well as arthritis.  It reduces the risk of heart disease by stimulating circulation, controls weight, and enhances emotional wellbeing.  </p>
<p>&#13;<br />
Maintaining a healthy diet is another way for post menopausal women to remain problem free.  They may want to think about supplements such as vitamin D and calcium to guard against osteoporosis. Not smoking may not only put off the menopause by a couple of years, it will also reduce the risk of heart disease and osteoporosis. Using progesterone creams may keep the vagina and bladder area healthier, and make intercourse more comfortable and therefore more appealing. </p>

There are many problems associated with post menopausal women, but by exercising regularly and maintaining a healthy lifestyle they don&#8217;t have to mean a huge decrease in quality of life.                </p>

<p>The Washington Post is reporting that a new federal study found that women who take a popular hormone drug after menopause can increase their chances of not only contracting breast cancer, but dying from the disease. The study followed more than 12000 women for 11 years and found that breast cancer was more common among hormone-users. This new finding by the Women&#8217;s Health Initiative comes after differing medical opinions on the use of hormones. For years, doctors have recommended taking hormones to alleviate symptoms of menopause and to slow aging. However, eight years ago, another Women&#8217;s Health Initiative study revealed that the risk of hormones, which included heart disease and breast cancer, outweighed its benefits. Since the publishing of this previous study, doctors have diverged on their recommendations, with some recommending very low doses for short periods of time while others have recommended use for up to 5 years. The new analysis suggests that women who took the combination of estrogen and progestin &#8211; sold as Prempro &#8211; were more likely to develop larger tumors that were more likely to spread to lymph nodes with an increased risk of death.

It&#8217;s become painful now, but avoiding it is now causing more problems with your spouse.<br />One problem after another comes into your life&#8230;one new alarm after another goes off. The final disaster in your life has happened &#8212; you are not yourself anymore.<br />One tiny emotional failure in your relationship can lead to drama in your life, but the measurement of this failure was inconspicuous and you hardly noticed it in the beginning. You wake up in the morning very sleepy and tired, cannot open your eyes and now you feel like you need your morning coffee.  You go to the bathroom to take a shower, look at the mirror and you see wrinkles on your face, wrinkles that you didn&#8217;t see before. You start putting creams and make up to cover them but every time gets more difficult to cover them and you feel like you are losing control of your life.<br />You go to your primary care physician, then to a psychiatrist, but do not get any answers.    But you need an answer now, because you are not yourself any more.    You were at a point in your life, finally, where you were happy with your work, with your status in life, with your family life, with your sex-life. Everything was running smoothly. For the past 40 or 50 years, you have treated your body right, and it, in turn has treated you right.<br />Until you start noticing little changes creeping in. Wrinkles? A little, for now. Mood swings? Slightly harder to deal with. Loss of energy? Causing a kink in your plans. Loss of libido? Bringing some major tension into the house. Is it the beginning of a disaster? What&#8217;s going on with your health?<br />Could it be one simple test that can prevent the disaster? Could your disaster start even before your first symptom?</p>
<p>Going through menopause or andropause is like trying to cross a stormy Atlantic ocean in a tiny boat. But nobody prepared you for this at all!</p>
<p>No one is able to explain to you why you are suffering and what needs to be done.    To your anger, your doctor, whom you love very much, says, that everything is OK with you. It is just aging, which is normal.    But you know something is disastrously wrong.    You have to get the answer now or you may die.<br />Official Western medicine does not give an answer to this question.    &#8221;There are no lifestyle changes, surgical procedures, vitamins, antioxidants, hormones, or techniques of genetic engineering available today with the capacity to repeat the gains in life expectancy that were achieved during the 20th century &#8220;  according to demographer S. Jay Olshansky of University of Illinois in Chicago as well as 51 longevity experts in their position statement in 2002.<br />That is why I had to launch my own investigation, based on scientific studies and my own experience. I did not have a choice but to do it, because I was going through andropause myself.<br />My investigation was based on facts and common sense, not controlled studies.    This method was used by doctor James Lind in about 1750 when he discovered how to fight scurvy. Six groups of two sailors were used. And one group which was given oranges and lemon &#8211; improved. The same method was used by doctor Edward Jenner in 1796, when he invented vaccination.<br />What would happen to the mankind if they would require double blind controlled studies to prove that vaccination is working for smallpox and lemon for scurvy?    We would be dead.    If the treatment is really working you do not need double blind controlled studies to prove it.<br />It is like your keys for your home door.    It is either fits or not. You do not need double blind controlled studies to figure this out. Normally an investigation like this is done by a group of scientists.    And it should be financed. Usually it is a big pharmaceutical company that invests money in the scientific study. And they did it before: they created drugs that mimic human hormones.</p>
<p>Because natural substances like hormones cannot be patented, they had to come up with something else. Instead of the most important women&#8217;s hormone estrogen they offered modified horse estrogens to produce &#8220;estrogen&#8221; for our women.    Instead of the second important women&#8217;s hormone progesterone they offered drugs, that sound like natural progesterone, but actually are synthetic &#8212; Progestins.    Why would they name the synthetic stuff like natural?<br />In the beginning everybody was happy: patients, because they felt better, doctors, because it was less work for them, pharmaceutical companies, because they were getting money.<br />Until Women&#8217;s Health Initiative (WHI) study came out.<br />Synthetic and semisynthetic hormone mimicking drugs Premarin and Prempro were jewels in the crown of the pharmaceutical company Wyeth. Their sales almost reached 1 billion dollars in 2001 1.    But even Big Pharma could not conceal the WHI damaging conclusions. What they found was that synthetic or semisynthetic hormones are linked to many life-threatening conditions like cancer and blood clots.<br />They are linked to cancer . They can cause blood clots. Low testosterone and decreased libido as well as gallstones are the complications of synthetic hormones, etc.<br />But the drugs were on the market already. And they were very profitable. So what they had to do was to warn patients about the risks, but continue to sell the drugs.    Who wants to stop money coming? But how did it happen that FDA approved drugs with life threatening side effects?<br />It is 2009. New York Times magazine Reporter Natasha Singer is looking into hormone replacement therapy drugs. But she is not interested in how those drugs were approved. After the scandals with Troglitazone or Trovan nobody is excited about it. She was interested in a much more important thing: how it happened that doctors recommended harmful drugs.    What they found was: it was ghostwritten medical papers, signed by doctors, that created medical &#8221; consensus&#8221;.<br />Can We Trust Such &#8220;Scientific&#8221; Articles?<br />I still remember my 27 year old female neighbor, who died because of a clot in her legs that went to her lungs. She was on birth control pills. Since then I decided not to advise traditional synthetic hormones to my patients.<br />So I arrived to my first solid conclusion:    no synthetic or semisynthetic hormones, because:<br />But do you need hormones at all? May be you can get back your life with diet and supplements? Or may be all you need to do is to bring your cholesterol down? Yoga? Meditation?<br />I still remember my first patient, suffering from heart attack: gasping, cold sweat, fear in his eyes and death imprinted on his face.    Or one of my cancer patients suffering from<br />excruciating pain.  Or Alzhemer&#8217;s patients, who know about their problem and are ashamed of it.  Maybe all we need to do is to get treatment for heart, cancer and dementia?</p>
<p>During my medical training I was told many times, that heart attacks are due to high cholesterol. What I did not know was about the Danish Eskimo study.<br />Danish scientists Bang and Dyerberg discovered that Eskimos almost never die of heart attacks, the number one killer, despite    the fact that they did not have that low cholesterol.<br />So it was not actually cholesterol, that kills! And they knew about it since the 1970s! It must be something else.    What about cancer? What about Alzheimer&#8217;s?    Swedish scientist from University of Lund Staffan Lindeberg, MD went to Kitava Island in New Guinea to study its 24,000 people population. What he found was that they do not have heart attacks! No cancer! No diabetes! No Alzheimer&#8217;s! But they die healthy on average at the age of 45 9.<br />So it is not cholesterol, it is not cancer, it is not dementia, that kills! But what is it then? I was at a dead end. And there was no help in sight. Do you think that Big Pharma will give away money to figure out how to get you better naturally?<br />Then I made my major breakthrough.<br />Seeing is believing.    Have you ever seen a person who looks twice as young as her real age? I did. She had beautiful skin and shiny eyes. Her dress could not conceal gorgeous curves of her body.    Her voice was getting to the bottom of everybody&#8217;s heart.    She looked like she was in her early 30s. In fact she was in her 60s. Her name was Suzanne Somers. Her Fountain of Youth was Bioidentical Hormone Replacement.<br />After many years in medical field, I had to admit that nothing like that could be achieved with traditional medicine.    So maybe what she is doing is right? Maybe what she is saying is correct: &#8220;It is not your cholesterol, it is not your blood pressure, it is your hormone decline, that kills you.&#8221;<br />Really? How about heart attacks, cancer and Alzheimer&#8217;s? When you develop chest pain when you are 20, it is pulled muscle. But if you have chest pain in you 70s &#8212; it&#8217;s a heart attack.<br />If you lose your key in your 20s, you&#8217;ll just get a new one.    But if you lose your keys in your 70s &#8212; it&#8217;s Alzeimer&#8217;s.    And you are right: we usually do not have heart attacks, Alzheimer&#8217;s or cancer in our 20s, when our sex hormone levels are high. But we do have them when our sex hormones are down.<br />Why?<br />Because mother nature does not need us that much after we cannot make babies any more. We need to free the space for young individuals, who can. Remember, that there is almost no menopause in the animal kingdom.  Fortunately, there is a loophole for humans: menopause and andropause.<br />Mother nature&#8217;s concept was simple: to ensure that humans are superior in survival compared to any other species in the world.    Therefore humans needed more than simple estrous cycle in animals. It was menopause in females and andropause in males, that changed everything.<br />Normally a fertile woman has her ovaries with plenty of eggs that are released in cycles. To get pregnant in the beginning of her menstrual cycle she produces sex hormone Estrogen, which makes the follicles with eggs to grow. When the egg is big enough, it is Follicle Stimulating Hormone and Luteinizing Hormone surge that, along with Estrogen, make the follicle break &#8212; a process called ovulation.<br />Then ovaries start producing another sex hormone Progesterone. Freed egg is then going to the uterus that is already prepared by Progesterone.    If there is a sperm there, then pregnancy commences.    If not, then egg is going out and Estrogen and Progesterone production ceases, uterus lining is gone, blood vessels become open and menstrual bleeding starts. Then the Estrogen production starts again, and uterus lining is restored.    Bleeding is over, and woman is ready for another cycle.    These cycles continue, until there are no more eggs in woman&#8217;s ovary. When there are no more eggs, periods stop and menopause begins.    Sounds simple.    But there is a problem here: an average woman has about 1 million eggs, but she uses only about 400.<br />Why are the rest of her eggs wasted?    Instead of using those eggs, women go into menopause and men go into andropause.<br />Why would mother nature invest in longevity but not in fertility?    The reason why still remains the mystery.<br />Maybe it is because mother nature wants a postmenopausal woman to take care of her daughter&#8217;s children so that she can make more babies? This is what Grandmother&#8217;s theory is saying. Or maybe that is because older males can compete with youngsters to<br />make competition more tough because older males are more experienced and sophisticated? This is what the Patriarch hypothesis is saying.<br />It does not matter why nature decided to give us menopause and andropause. What matters is: we do have a loophole to live longer. The only thing we have to do is to pretend that we are still young. But how?</p>
<p>1930, Canada, McGill University, Department of Biochemistry.<br />A young Chairman, Professor Dr. James Collip, who just co-discovered Insulin, was thinking how to get relief of menopausal disaster. After series of experiments and failures he eventually found the solution. What he did was: he extracted estrogen from the urine of pregnant women and gave it as a menopausal remedy. And it did work. Because giving natural hormones of fertile women to postmenopausal individuals was a signal to their bodies that they are still fertile. That is how we can pretend that we are still young. What you did not realize was: she is the perfect example of a woman, full of Estrogen: confidence, strong desire to have sex, big breasts and low waist to hip ratio.  That is why estrogen is sometimes called &#8220;Marilyn Monroe Hormone&#8221;.<br />Estrogen plays a critical role in a human&#8217;s body. It is responsible for breast development, vagina lubrication, burning fat, maintaining your bones, skin and vessels, bringing your good cholesterol up and bad cholesterol down, etc. Because it works as an antioxidant, it protects your brain. No estrogen &#8212; mood problems, memory problems, etc. But estrogen alone is not enough.<br />Confidence and strong sexual desire should be balanced, otherwise they will go too far. And the hormone, that does the job is Progesterone.<br />Have you ever seen pregnant women? They are calm because they have plenty of Progesterone. Do you know, that a cup of milk in the evening may help to get better<br />sleep? There should be something, that gives us the energy and sense of well-being, as well as ability to maintain our muscles, mental and muscle energy and libido. It is hormone Testosterone that does the job.<br />1889, Paris, France. Aging former Harvard professor and now Professor of Experimental Medicine of College de France Doctor Charles &#8212; Edouard Brown &#8212; Sequard, is looking for an elixir of youth.    He is a shrewd observer and a talented scientist. His name is immortalized in the modern neurology by the spinal cord damage syndrome, bearing his name. But he was aging as anybody else. He was desperately looking for a remedy, and he found one, later called Brown-Séquard Elixir. What he did was: he self injected an extract from guinea pigs testicles. According to him his vigor was back, his sense of well-being was back. His Elixir was Testosterone.  It was the famous medical journal, The Lancet, that first published his observations.<br />Do you think he was prized for his discovery? Actually his colleagues were laughing at him. It took them 40 years to rediscover the importance of Testosterone.  Now we know, that it is not only for sex drive, erections and muscles. It is also to maintain you bones and protect you from Diabetes, Alzheimer&#8217;s, heart disease etc.<br />Bottom line: we need all of them: bioidentical Estrogen, Progesterone and Testosteone ( women and men &#8212; both ).</p>
<p>Not a single other treatment ignited so many debates and controversy, as Bioidentical Hormone Replacement Therapy ( BHRT). The official medicine including FDA, International Menopause Society, American Medical Association, American College of Obstetricians and Gynecologists, Mayo clinic, Endocrine Society, etc.,  is saying, that Bioidentical Hormones carry the same risks as synthetic hormones and there is no evidence of any other benefits of Bioidenticals.<br />I wonder, why so perfect a consensus is about it. Recently two branches of Government Research were trying to figure out if Chronic fatigue syndrome is associated with with exotic XMRV virus. One group (Harvey Alter, FDA and National Institutes of Health) found that yes, another group (William Switzer, CDC) found that no.<br />If the scientists, who use the same methods and testing the same virus, cannot come to the same conclusion, then how could they be so unanimous in much more complicated subject as Bioidentical Hormones?<br />Also, the FDA is saying that risks of Bioidenticals are the same as synthetic. Based on what?   Did they test Bioidentical Hormones? Not that I know of. They assumed, that Bioidenticals work the same as synthetic, which we know are harmful.<br />It is like saying that one dollar bill has the same value as 100 dollar bill, because they are the same size.<br />On the other hand the proponents of Bioidentical Hormone Replacement Therapy are saying, that Bioidenticals are safe and have no side effects.<br />Two French studies, published in 2008, suggest, that Bioidenticals are safer, that conventional BHRT. According to Dr. Kent Holtorf&#8217;s literature review &#8220;Physiological data and clinical outcomes demonstrate that bioidentical hormones are associated with lower risks, including the risk of breast cancer and cardiovascular disease, and are more efficacious than their synthetic and animal- derived counterparts. Until evidence is found to the contrary, bioidentical hormones remain the preferred method of HRT. Further randomized controlled trials are needed to delineate these differences more clearly.&#8221;<br />Even some prominent clinicians have to admit publicly the advantages of Bioidenticals. According to Dr. Christine Derzko, Chief of Endocrinology at St. Michael&#8217;s Hospital in Toronto, &#8220;Despite the weak evidence, some promising preliminary data are emerging, suggesting that pharmaceuticals containing BH ( Bioidentical Hormones &#8212; Sergey Kalitenko ), estradiol and progesterone, have equal (and in some cases, lower) risk compared with other CHT ( conventional hormone therapy &#8212; Sergey Kalitenko ).<br />Normally we need controlled randomized studies of the efficacy and safety of Bioidentical Hormones.    But there is no such study in sight.    Do you think Big Pharma will invest money in something, that cannot be patented and therefore will unlikely to be profitable?<br />But what do we do, if we do not have long term studies so far? Let see what experts say: &#8221; I always tell clinicians, that until we have definite randomized trial data &#8212; which we may never have &#8212; whatever patient wants to do is the correct decision&#8221;, &#8212; says Dr. Speroff, who is a Professor of Obstetrics and Gynecology at Oregon Health and Science University and a sub-specialist in reproductive endocrinology &#8211; one of the most respected OB &#8212; GYN doctors in the US,<br />He is the founder and recently stepped down as director for the Women&#8217;s Health Research Unit at OHSU. Doctor Speroff has gained prominence in the area of women&#8217;s health throughout his publications, national and international lectures and extensive work on clinical trials.<br />But do Bioidentical Hormones really work? Do they really prolong life? Or may be they are another myth? I was desperately looking for the answer, but could not find the right one.</p>
<p>My next breakthrough came unexpectedly &#8212; from Blue Zones study.<br />Because official medicine does not give us the longevity prescription, not doctors, but National Geographc reporter Dan Buettner decided to look for the areas on Earth, where people live longer, than in the other areas. They decided to learn from real people, not from theory, how to live longer. But what did they find?</p>
<p>Longevity lessons from real long living people for Blue Zones:</p>
<p>1. Get enough sun exposure.</p>
<p>2. Eat nuts and tomatoes</p>
<p>3. Be social.</p>
<p>4. Restrict your calorie consumption.</p>
<p>5. Move naturally.</p>
<p>6. Drink enough water.<br />Sounds like nothing unusual. But somehow together they worked much better than any other protocol suggested. What was the secret behind Blue Zones lessons?<br />The solution came unexpectedly. While investigating about tomato benefits, I learned that the most valuable their component antioxidant lycopene is going to the testes, adrenal glads and liver.  Looked like tomato are protecting endocrine glands from oxidaive stress, but for what?   To make sure that they make enough hormones!    And I said to myself: &#8220;Gee! This means, that she had enough Estrogen to maintain her bones!<br />Another centenarian, from Nicoya, named Rafael Angel Leon Leon, had multiple girlfriends until age 94, then he married a woman, who was 40 years younger then he was! Nobody marries an older person who cannot have sex! Therefore Mr. Rafael Leon had enough testosterone to have sex!<br />Looks like longevity is all about hormones. But what about safety?<br />It is like trying to answer the question if drinking water is safe. Yes, but in moderation. If you drink it too much your brain will swell to the extent that you may die.<br />Obviously estrogen itself is not a problem, because your body cannot produce poisonous hormone.    The problem comes when it is too much estrogen, that leads to increased cancer risk or it is converted into poison, that causes cancer.<br />Normally the main body estrogen &#8211; estradiol or E2, is removed from your body very fast by 2 ways:<br />1.  It is converted into 2 OH estrone, which is benign.</p>
<p>2. Estradiol binds to bile in the gut, becomes water soluble and then is eliminated via<br />kidneys. So estradiol is removed. No poison made. Danger is over.<br />But on 3 conditions:</p>
<p>1.    Your body knows what to do with hormone.</p>
<p>2.    There are enough enzymes to do the job.</p>
<p>3.    Your gut functions properly and does not have wrong bacteria in it.<br />If even a single condition is not met &#8212; you might be in danger.<br />Your body does not know that to do with synthetic and semisynthetic estrogens. Therefore instead of 2 OH estrone they are transformed into 4 OH estrone, which leads to free radical formation and therefore cancer.<br />Bioidenticals mostly do not have this problem, but what if your body does not have enough enzymes? Or your gut is not functioning properly?<br />Fortunately we can monitor what your body is doing with estrogen and most of the time make it right.<br />Bottom line is simple:<br />Get more energy, better sleep, mood, productivity and sex life with Hormone Balancing. You can get better with Bioidentical Hormone Replacement Therapy ( BHRT ), but be aware of the risk involved. Doing nothing does not bring you anywhere. What can help you is finding the root cause of your problem and solving it.<br />Bioidentical Hormone replacement is very effective and reasonably safe.<br />To me it is more than using Bioidenticals, it is about using a holistic, individualized approach instead of standard protocols.    To me it is about using natural remedies instead of potentially toxic drugs. <p>3.    Eat plenty of tomatoes.</p>
<p>4. Move naturally. Remember monkey in the gym?</p>
<p>5.    Eat handful of nuts regularly.<br />6.    Drink enough water.</p>
<p>7.    Be social.<br />My suggestions:<br />1.    Have enough sleep (at least 8 hours a day).</p>
<p>2.    Follow Paleolithic diet &#8212; avoid processed food!</p>
