This issue I thought I’d digress from my usual “do it yourself” topics to something you should not “do yourself” – the use of estrogens by transsexuals. Most of this admittedly technical information I obtained from research at the Santa Clara Valley Medical Center library, though literature is scarce on the subject even there.

Estrogens are powerful steroid hormones, chemicals which affect the form and function of the body and its organs.

There are three basic human estrogens: estradiol, estrone, and estrial. Estradiol is the most active form and estrial is the least active. In women, large amounts of estrogen are produced by the ovaries, and in men a small amount is present due to chemical conversion of testosterone.

Once you are grown and genetic male traits are fully developed, the only way your body organs have of knowing what sex you are is by the levels of male and female hormones which are present. Changing the hormone balance from male to female with drugs causes tissues which are supported by male hormones to diminish and stop functioning and those which are supported by female hormones to develop and begin functioning.

If your doctor has prescribed estrogens for you, it is probably in the form of Estinyl (ethinyl estradiol), Premarin (conjugated estrogens, a mixture of the three estrogens plus estrogen breakdown products) or injections (such as estradiol valerate). Though it would seem to be desirable, no suppository form of estrogen is available except as in the form of D.E.S, a drug which is not in favor today.The most powerful single oral dose is the 0.5 milligram Estinyl tablet, but faster results can be obtained by using two or even all three of the Estinyl, Premarin, and injections. Any of the three will produce in time a certain amount of bodily feminization. (Note: Premarin alone has been shown to be unable to reduce blood testosterone levels to a female normal, though Premarin alone does produce feminizing effects, albeit slowly.

Also: generic Premarin has become suspect as to its quality, potency, and purity – many pharmacists discourage use of the generic, or suggest that dosages of the generic be increased relative to the dosage of the brand name Premarin product.) The effects include breast development (usually slight to average development; occasionally nearly none or quite a lot, depending on genetics and body type), reduction in size and firmness of the testicles and prostate gland, some reduction and repatterning of body hair, softening of the skin, recontouring of the body due to accumulating layers of feminine body fat, a considerable reduction or elimination of (masculine) sex drive, and improved effectiveness of facial hair removal by electrolysis.Testosterone levels in the blood drop to very low levels due to effects of estrogen on the brain and directly upon the testicles. Since testosterone tends to fuel the male emotional characteristics of aggression and competition, many patients report feeling more mild or tranquil. Reduction of male hormone levels may also clear up acne and excessively oily skin. Little or no changes in voice quality can be expected, though sometimes a slight increase in range is noted.

The cost of oral hormone supplements is not excessive. Typical prices are: Provera 10 mg. 100 units, $ 40 (generic medroxyprogesterone HCL is much less.); Premarin 2.5 mg 100 units, $ 45 (generic less but not recommended); and Estinyl 0.5 mg 100 units, $ 55 (no generic available). Injections may run $ 15-40 plus office visit charges. Black market prices for the above begin at about three times the pharmacy cost.

Choice of an endocrinologist is best made by personal referral, either by a friend or therapist. The regimen and requirements of doctors varies widely, as does their level of experience in this very specialized field. Many doctors require concurrent counseling by a psychiatrist or psychologist.

If you experience any dissatisfaction with your therapist or doctor, a consultation with another may produce different results. There is little concrete knowledge of transhormonal therapy in the medical profession, and research on the subject is scarce – your doctor’s expertise is probably mostly due to his or her experience. At the very least, your doctor should have good general experience in the administration, effects, and side effects of female hormones, and be aware of the Standards of Care (the Harry Benjamin International Gender Dysphoria Association criteria for surgical and hormonal treatment of transsexuals.)

Your doctor may be cautious in prescribing large amounts of estrogens to you if you have any of the following history or symptoms: high blood pressure, any heart disease or defects, clotting disorders such as phlebitis, stroke or cerebrovascular disease, liver function abnormalities, a history of heavy alcohol intake, kidney disease, migraine headaches or seizures, diabetes, family history of breast cancer, obesity, or heavy smoking.

Periodic checkups with your doctor are required to spot early signs of certain dangerous conditions. Among these are: benign or malignant tumors of the liver, breast, pituitary gland (in the brain), and kidney, along with phlebitis and elevation of blood pressure. Heart attack (myocardial infarction) and stroke have been reported in relatively young transsexuals receiving estrogens, especially those with clotting disorders. Changes in a part of the prostate gland known as the verumontanum can cause blockage of the urinary tract after long term use – this must be corrected surgically. Lactation or discharge from the breasts can be a sign of a potentially dangerous pituitary gland condition.

Your doctor will administer periodic blood tests and may check the following: testosterone (should be less than 85 nanograms per 100 milliliters), prolactin (should be less than 45 ng/ml), liver function scans, and clotting time. He may also feel your breasts for lumps and listen to blood flow in your major veins and arteries.

Once you have been using estrogens for a year or more, some effects may become irreversible even if estrogen intake is ceased. Certain chemical processes in the brain remain in a female pattern permanently, and changes in brain wave patterns have been reported. These effects may or may not be associated with emotional and personality changes. Breasts and female fat distribution may not subside after administration of estrogens, and sex drive may remain relatively low. For these reasons, it is important to be certain of your committment to feminization of your body.

Sudden changes in dosage of estrogens, either increasing or decreasing, have been known to produce severe mood changes. The effects may be likened to going through menopause, puberty, and pregnancy at the same time. Lethargy, depression, anxiety, difficulty in concentration, headaches, abdominal cramping, nausea, and other symptoms have been noted for periods of days or weeks. It may be wise to change dosages as gradually as possible. (Despite what your doctor may tell you!)

Changes in metabolism are common, with weight gain, water retention, and increased appetite as the major effects. Estrogen reduces the ability of the body to eliminate certain drugs such as Valium so that smaller dosages of these medications become as effective as larger dosages were before. This is also true for alcohol so be sure to reassess your limits – this explains why the tolerance for alcohol of women is typically less than that of men. Any physician you deal with should know of any medications you are taking – with surgery this can be critical due to the effects of estrogen on the blood clotting rate.

After genital surgery, estrogen doses may be greatly reduced if the desired degree of feminization has been achieved. Since the testicles are now absent, it is no longer necessary to suppress testosterone production. Risk factors are believed to be in proportion to dosage, so the minimum effective dose is preferred for long term use. This means reducing dosage by a factor of one fifth to 1/20th of previous levels.

Risks of estrogen use can be minimized by having injections alone. Injections of Delestrogen, Estradurin, etc. cause the estrogen to enter the bloodstream directly, without the first pass through the liver. This means the liver works much less hard in metabolizing the estrogen, and can return to doing the normal work that the liver does in digesting food and eliminating toxins. Injections are given deep into the muscle tissue of the buttocks, once a week to once a month. The effects are similar to the higher doses of oral hormones, and sometimes it appears that feminization progresses further with injections than with orals. If the injections are done at a doctor’s office, the costs may be about equal to the cost of oral pills – but individual doctors’ rates and charges vary a lot in this area.

Generally, an endocrinologist who prescribes injections can be persuaded to teach you to administer the injections yourself, with a short training session. By doing the injections yourself, and buying generic versions of the injectables, you can save up to 90% of the costs of oral hormones, making this by far the least expensive alternative. If you are going to be using hormones the rest of your life, and wish the safest, most effective, and cheapest method, then make self-injection your goal.

It is thought that estrogens should be taken along with a progestin (a chemical with effects similar to progesterone) such as Provera (medroxyprogesterone). A progestin will tend to maximize breast development due to enlargement of the milk sacs themselves (as opposed to breast fatty tissue) and will approximate more closely the natural female hormone balance. A more natural hormone balance may provide some shielding against some of the hazards mentioned previously. Also progestins greatly reduce male sex drive. I could find no agreement in the literature as to the recommended dosage but higher dosages seem to pose no known risk.

REAL progesterone in the form of capsules is now available, but rather expensive ($ 1.25 to $ 5.00 per daily dose). Real progesterone is available as an injection also, at a very low price – the disadvantage is that the effects last only 3-4 days, so an injection twice a week might be called for. Real progesterone has ALL the benefits of progesterone, instead of only some of them as with progestins. Most doctors who give injections use Delalutin (hydroxyprogesterone caproate) instead of real progesterone, but the TS giving herself her own injections might consider using real progesterone instead, because of the reduced costs.

Proper medical management of estrogen administration can reduce the hazards and maximize the benefits of transhormonal therapy. It makes good sense to know the facts and follow medical advice when using these powerful drugs.

Testosterone is the primary hormone that gives men their “male” characteristics. Once a boy hits puberty his body gets flooded with testosterone and he becomes a man. Testosterone is responsible for the proper development of male sexual characteristics. It is also important for maintaining muscle bulk, adequate levels of red blood cells, bone density, sense of well-being, and sexual and reproductive function. On the flip side, women have estrogen, which is the female “equivalent” of testosterone. In women, estrogen is what is responsible for the proper development of female sexual characteristics.

So men have testosterone and women have estrogen, right? Wrong. Men actually have testosterone and estrogen-women have both too. The difference is that men produce about ten times more testosterone than women do (and vice versa). So though our bodies are dominant in testosterone, we also produce low levels of estrogen.

In fact, a certain amount of estrogen in men is necessary for proper body function. Too little estrogen and we run the risk of developing osteoporosis and other conditions. Too much though and we start developing female characteristics, which is not a good a thing-for health or the ego.

As men age their testosterone levels will naturally start to drop (by about one percent per year). Similarly, their estrogen levels will start to increase.

When estrogen levels start getting high in men, health problems start to arise. Elevated estrogen levels in men have been shown to contribute to prostate cancer and heart disease, along with gynecomastia-also known as “man boobs.” Since higher levels of estrogen are typically accompanied by lower levels of testosterone, other changes occur including loss of muscle mass, fatigue, low libido, erectile dysfunction. Lastly, excessive estrogen in men raises body fat and can contribute to diabetes and high lipids.

But it’s not just older guys who experience higher levels of estrogen-it happens to young guys too. Guys who don’t take care of themselves are far more likely to show elevated estrogen levels than men who are in good shape. For example, testosterone begins to convert to estrogen in men as they age due to the aromatase reaction.

Aromatase is an enzyme that plays a role in converting testosterone to estrogen and is found most prevalently in fat cells. Therefore, the more body fat a man has the more aromatase and the more estrogen he is likely to have. So you see, a younger guy who is overweight with excess fat-especially around the middle-will likely see a spike in estrogen levels accompanied by a drop in testosterone levels.

Rule number one to avoid excessive estrogen levels: maintain a healthy body weight-build and maintain lean muscle mass. In particular, pay close attention to the fat around the middle of your body. These fat cells are known to produce aromatase, which can lead to higher estrogen levels in men, thus changing the hormonal balance.

There are plenty of other things that can lead to higher estrogen levels in men too so guys of any age should be aware of the following:

* Medications: Some medications can cause estrogen levels to rise so be on the lookout for estrogen-containing drugs, steroids, ulcer medications such as cimetidine, some antibiotics (tetracycline, ampicillin, etc.), anti-fungal medications and antidepressants. Since there are more medications that can cause this effect, you should check with your doctor or pharmacist.

* Illness: Certain illnesses also cause spikes in estrogen levels including some tumors of the testes, adrenal and pituitary glands, liver and lung, cirrhosis (liver disease). Kidney and thyroid disease may elevate estrogen levels as well.

* Drugs: Using illicit drugs is another sure-fire way to cause a jump in estrogen so stay away from anabolic steroids, marijuana, cocaine and other drugs that can affect hormone levels.

* Alcohol: Moderate drinking generally doesn’t cause problems but frequent benders can. Drinking excessively can increase the body’s conversion of testosterone into estrogen, especially in fat cells. It can also block the liver from effectively eliminating excess estrogen.

* Environmental exposure: Things that we run across in our daily lives can also contribute to elevated estrogen levels in men. The list of pesticides, chemicals and herbicides that have the potential to cause estrogen-like effects in the body is lengthy so do yourself a favor and avoid them as much as possible.

A number of experts have also begun to believe that there is a link between the ingestion of hormone-enhanced food and elevated estrogen levels. More research is sure to be done on the subject so stay tuned. In any case, hormone- and pesticide-free, unprocessed foods are best for your body so stick with those.

I always like to start out by stating I am not a doctor, I am not offering cures or medical advise. I am a concerned citizen who is interested in researching alternative options for health and wellness.

Since the 1930′s, American women have been trained to think that a “natural” part of growing older is Menopause, this “disease condition” requiring treatment. If Menopause is a disease, how did millions of women go through history without Hormone Replacement Therapy (HRT) drugs to deal with it? How do women in third world countries handle going through it without HRT’s?

Diet plays an enormous part in how we “go through” Menopause. For instance, Shangri-La place in the Himalayas called Hunza Land, famous for longevity to 120 years old, symptoms of Menopause were unheard of.

Natural Phytoestrogens (plant-estrogens) are found in plants like, licorice, soybeans, alfalfa, and many others. Phytoestrogens are weak estrogens and have the ability to block the stronger forms.

In 1966 a best seller, Feminine Forever, by Robert Wilson, a New York Gynecologist, explained that Menopause is an Estrogen- dificiency disease. Insufficient Estrogen supposedly caused women to lose her youth, beauty, cheerful attitude, vaginal dryness, hot flashes, bone density, all at once, with the onset of Menopause. The drug industry in a heart beat donated .3 million to set up the Wilson Foundation for the sole purpose of developing and promoting Estrogen drugs. The FDA’s approval for sythetic Estrogen was given based on one study. Throughout 1964 and 1965, fueled by advertising powers, Estrogen Replacement Therapy (ERT), a synthetic Estrogen was being popularly prescribed.

In 1975 a little snag came up, findings of increased Endometrial Cancer cases had gone up by 5 times. Unless they had been using the drugs for longer than seven years, then it was 14 times the normal incidence.

WHAT IS ESTROGEN?

Estrogen is really a general term for three seperate Hormones, Estriol, Estradiol and Estrone.

Estrogen is produced in three main places in a woman’s body, in the Ovaries, Adrenal Glands and the Fat Cells. 

What is Estrogen’s Main Purpose? The main purpose of Estrogen is the make the uterine lining, the endometrium, ready to implant a fertilized egg in the event fertilization occures. To aid in this function, Estrogen will promote…water retention, fat storage and maturation of the female adolescent. This is all fine, if pregnancy is likely, but EXCESS Estrogen throws off the timing. Excess Estrogen causes the body to prepare for embryo implantation all the time. This state of over-preparation is the cause of , sluggish blood circulation, migraines, increased clotting, high stroke risk, fibroids, endometriosis, weight gain and a host of other conditions as well as disrupted copper/zinc ratios in brain cells attributing to mood swings.

Estrogen has a sister hormone called Progesterone, whose functions are equally important.

What is Progesterone? Progesterone is the other primary female hormone. It is produced in the ovaries. It is the precursor for both Estrogen and Testosterone. Progesterones functions include, maintaining the endometrium in pregnancy, new bone formation, regulation of blood pressure, fat conversion, sugar metabolism, maintaining myelin sheath (nerve insulation) and regulates the Estrogen Production.

When an egg is presented once a month from the ovaries, wrapped in an envelope called a follicle. After the follicle lets go of the egg, the egg journeys down the Fallopian tubes on its way to the uterus, where it awaits possible fertilization. The burst follicle still has an important job to do: it begins to produce Progesterone, for the next two weeks. Progesterone’s job is to maintain the uterine lining until one of two things happen. Pregnancy or no Pregnancy, if Pregnancy occurs Progesterone production is taken over by the developing lining itself – the Placenta. If no Pregnancy occurs, the follicle stops producing Progesterone, which triggers the collapse of the blood-rich lining, which is then expelled as the woman’s monthly cycle.

So, these two hormones are responsible for entire infastructure of reproduction. So what then is the problem?

Back to Estrogen Dominance. If Estrogen is too high, Progesterone cannot keep the dynamic of balance. How does Estrogen get too high? Overrefined diets, external toxins from the environment, (xenoestrogens), and lack of excersise. These can raise abnormal levels of Estrogen. Lets focus on Xenoestrogens, or “foreign” Estrogens. Many external Toxins have been found to have Estrogen-like effects in the body, most are from Pesticides and Fertilizers, Petroleum Derivatives, Plastics, Soaps, (foaming agents, hormone laced meat and milk products, etc. This affects everyone on a daily basis, men, women and children. The research has shown that high levels of Estrogen in Men show Gonad shrinkage as well as up to a 50% drop in sperm count, in Women, overdeveloped Ovaries as well as the host of problems listed earlier and in children early onset of Puberty. Progesterone is simply overwhelmed by the dominance of Estrogens.

As Estrogen levels build up to twice the normal level, many systems of the body are adversely affected. Our Body Fat stores Estrogens. Estrogen Dominance can cause Sugar Metabolism Defects leading to Diabetes, Endometrial Cancer is increased by 5-14 times, Osteoporosis, Toxemia ( blood poisoning), leading to autoimmune disorders such as, Lupus, Chronic Fatigue, and Arthritis. The increase risk of Stroke is 50% with extrogen, according to an extensive project, known as the Boston Nurses Questionnaire Study, of 121,000 nurses. Estrogen Dominance can lead to Fibroids, Breast Cancer and it has a detrimental affect on B Vitamin depletion, leading to fatigue, sluggish memory and hair loss. “The only known cause of Endometrial Cancer is unapposed Estrogen,” Dr. Lee, author “What Your Doctor May Not Tell You About Menopause.”

A growing number of medical researches who do not represent interests of the drug cartels are stepping forward to show that the symptoms of menopause are not caused by too little Estrogen, but by too much. American women arguably are among the most stressed people in history, emotionally and nutritionally, as well as environmentally. It is well known that stress depletes the Adrenal Glands. When this happens, Progesterone is converted to Adrenal Hormones, like Cortisol, to take up the slack and try to keep up with Adrenal demands. Remember Progesterone is their precursor. The result is further depletion of Progesterone, again promoting Estrogen Dominance.

Hormone Replacement Therapy, is this the right option?

There are MANY side effects of HRT (Hormone Replacement Therapy), those of you who have either been on them or researched them know the whole list of possible side effects, many deadly. Synthetic Drugs, even ones that include “Synthetic Progesterone” is not handled by your body like a “Natural Progesterone,” one that is bio-identical to your bodies own Progesterone Hormone.

What Solutions are available to combat Estrogen Dominance?

Researchers have also discovered that estrogen can be metabolized in two different pathways in the liver, which results in two different kinds of estrogen, “good” and “bad” estrogen. “Good” estrogen metabolites are released into the bloodstream there they cause many of the benefits estrogen provides, such as prevention of heart disease, and strong, healthy bones. “Bad” estrogen, in large quantities, can result in many health problems including autoimmune disorders, breast cancer, cervical dysplasia, clinical depression, fibroid tumors, infertility, magnesium deficiency, osteoporosis, ovarian cancer, uterine cancer, and zinc deficiency.

When dealing with estrogen dominance, it is not a good idea to take more progesterone to even out hormonal balance. Taking more progesterone won’t necessarily restore hormonal balance. Rather, it might be metabolized into more estrogen, resulting in an even greater hormonal imbalance and greater estrogen dominance. Instead, one should take the DIM supplement, which improves hormonal balance and eliminated estrogen dominance by shifting the way that estrogen is metabolized in our body. This creates more “good” estrogen metabolites and less “bad” estrogen metabolites. Although it does not increase or decrease estrogen levels, it directs how the estrogen is metabolized.

When buying a DIM supplement, it is suggested that you read the label of the DIM product you’re going to buy. Be certain that you are getting 120 mg of DIM complex, which is standardized to 30 mg DIM per dose. Because DIM is very hard to absorb, the products needs to be in a specialized complex that improves bioavailability.
